Defects are items that are not conforming to expectations or customer specifications. They are waste, as they lead to scrapping, extra efforts for rework and/or re-inspection, and to additional documentation, thus meaning a loss of time and money.

Examples:

  • A burnt toast for breakfast
  • A bug in a software
  • A hole drilled with the wrong diameter
